Background and purpose: Attention Deficit/ Hyperactivity Disorder (AD/HD) is one of the most common
neurodevelopmental disorders, characterized by deficits in core executive functions such as attention,
response inhibition, and working memory. Given the importance of improving these functions, the use of
cognitive rehabilitation programs has gained attention as a novel approach to therapeutic interventions.
This study aimed to investigate the effectiveness of the Captain’s Log cognitive program on improving core
executive functions in children with the combined type of AD/HD.
Method: This study was a single-blind controlled clinical trial using a pre-test-post-test design with a
follow-up period. The statistical population included children referred to the Atieh Center from 2022 to 2024.
34 children with combined AD/HD were selected by convenience sampling and randomly assigned to two
experimental groups (n=17) and control groups (n=17). The experimental group practiced with the Captain
Log program for 10 sessions, and the control group practiced the Tetris video game program. Participants
were assessed in three stages: pre-test, post-test, and follow-up with continuous performance (CPT),
N-back, and Integrated Visual and Auditory–2 (IVA-2) tests. Data were analyzed using SPSS software and
multivariate analysis of covariance.
Results: The results showed that the performance of the experimental group in attention (F=70.39, P<0.001),
response (F=48.62, P<0.001), and working memory (F=57.89, P<0.001) significantly improved compared to
the control group. However, there was no significant difference between the two groups in any of the variables
at the follow-up stage (p>0.05).
Discussion and Conclusion: The Captain Log rehabilitation program can be used as an effective tool in
improving core executive functions in children with AD/HD in the short term. However, to achieve sustainable
effects, there is a need for supplementary interventions and continuous training.
